Infectious enthusiasm goes a long way here.Please view the extraordinary story of Giro’s Persuasive Powers .The Role:As a Team Manager for our exceptional Pathway 1 service which sits within the Bi-Borough Hospital Discharge Team, you can make your own powerful contribution to residents who live in Westminster, Kensington and Chelsea. Supporting the Head of Service in leading this innovative initiative, you will work with elderly and vulnerable adults who are medically ready to be discharged from hospital.With oversight of the day-to-day activities of four Social Workers, you will see to it that the discharge service is aligned with numerous other partners such as Acute Hospital Trusts, the Community Independence Service and local CCGs. Not only that, but you will build successful partnerships with voluntary organisations and those involved in the delivery of mental health provisions, identifying how these teams could work together better in order to support as many people as possible.With a large population to take care of, there is a great deal of pressure on our services, so we are collectively committed to ensuring that nobody falls through the net. But with your calm leadership skills and ability to promote the most efficient ways of working together, we will be able to get people back into their preferred environments, as quickly as possible without ever compromising on their safety or wellbeing. As such, we will expect you to ensure the team are meeting the relevant policy and legislative requirements, so safeguarding and the Mental Capacity Act will underpin much of what you do – though you will always strive to personalise the services that we are offering to our residents as they leave hospital.Working across numerous key sites, this fast-paced role will provide you with plenty of variety, plus the chance to make a tangible impact on our residents’ lives every day – making this an exceptionally rewarding opportunity to further your career with the support of a friendly and ambitious team.Please refer to the for more informationAbout You:First and foremost, we will expect you to take genuine pleasure in helping people secure a safe and secure home environment once they have been discharged from hospital. You will therefore be both compassionate and astute, understanding when (and how) to safely discharge an adult, and when to provide our patients with alternative options if the risks outweigh the benefits.With either a Social Work, Nursing or Occupational Therapy qualification, you will have at least three years post-qualification experience, with one year in a supervisory or management role. As an inspirational leader with the ability to recruit, support and develop the skills of colleagues, you will be capable of influencing senior practitioners and staff, and you will know how to successfully implement large scale change that will benefit our local residents.
