Headquarters Negotiable within the Pee Dee Region between Conway, Florence or Horry Counties. This position is eligible for partially remote or adjusted work hours after six (6) months of employment.Is responsible for providing therapeutic nutrition services for clients with high risk and/or complex nutrition-related medical conditions. Develop nutrition care plans for high risk patient referrals and approve special and metabolic formulas. Participate in quality assurance processes for nutrition services. Provides care coordination to Children and Youth with Special Health Care Needs (CYSHCN) clients in a public health setting.Care Coordination & Planning: Responsible for providing therapeutic nutrition intervention for clients with high-risk and/or complex nutrition related medical conditions. Facilitates the coordination of care for diverse Children and Youth with Special Health Care needs (CYSHCN) clients, liaising with Women Infants and Children department (WIC), nursing, therapists, physicians, schools, social workers, and other relevant providers. Ensures caseload management adheres to program policy and procedures.Nutrition Care: Assess and diagnose the nutritional status of clients with high risk and/or complex nutrition-related medical conditions as referred from WIC, Maternal Child Health (MCH) programs, and other health care providers. Develop and implement nutrition intervention plans to include counseling, follow-up, referral, and coordination of care with other health care providers. Provide and document care utilizing the Nutrition Care Process (NCP) and according to program policy. Develop nutrition care plans for high-risk patient referrals and approve special and metabolic formulas.Formula Tracking and Distribution: Approve, document, distribute and track special and metabolic formulas per program policy and procedures.Consultation, Technical Assistance and Outreach: Provide professional expertise, consultation and technical expertise and assistance to health care team members, health care providers, community organizations, staff and the public. Participate in the orientation and training of nutritional staff as requested. Serve as a preceptor for dietetic interns completing supervised professional practice experiences in the region.Quality Assurance: Assist in the planning, implementation, and evaluation of MCH nutrition services. Assist with implementing quality assurance processes for MCH program nutrition services.Professional Development and Training: Maintain ongoing professional development to ensure delivery of quality nutrition services to include participating in continuing education credit and training opportunities.Other duties as assigned.State Minimum Requirements: Some positions require registration or eligibility for registration by the American Dietetics Association. A bachelor’s degree in nutrition, dietetics or community nutrition and experience in a clinical, public health or institutional setting; or a bachelor’s degree in any field, eligibility for registration by the American Dietetics Association and experience in a clinical, public health or institutional setting.Agency Additional Requirements: Registered dietitian (RD) with a master’s degree in nutrition, community nutrition, dietetic, public health or related field; or a Registered Dietitian (RD) with a bachelor’s degree in nutrition, community nutrition, dietetic or related field and two (2) years’ experience in clinical dietetics or as a nutritionist in a public health or community setting providing clinical service. Must have a current license to practice dietetics from the SC Panel of Dietetics or be eligible to apply for a license to practice in South Carolina.DPH employees are required to participate in disaster preparedness during hazardous weather or declaration of an emergency. This involves being subject to duty and or call on a 24-hour basis.
You must possess a valid driver’s license and have access to a reliable transportation.
This position may require working early (before 8:30 am), evening (after 5:00 pm) and/or Saturday’s.
