Job introductionAbout the roleDo you want to work in a multi-disciplinary team supporting adults with multiple compound needs?A fantastic opportunity has arisen for a Senior Practitioner to join the Changing Futures Multi-Disciplinary Team, on a fixed term contract for 12 months within East Sussex County Council – Adult Social Care – Mental Health Service.This is a countywide role with the office base at St. Mary’s House in Eastbourne. Hybrid working arrangements are applicable to this role, which will include elements of office-based working, client visits and working from home.The Changing Futures Sussex Programme focuses on meeting the needs of the most vulnerable people who repeatedly fall through the net, working alongside people with lived experience of multiple disadvantage, to achieve positive changes in services to make them better connected and easier to access.The Changing Futures Programme has been awarded funding to continue for a further 12 months until March 2026. The Programme aims to create an environment where individuals experiencing multiple disadvantage can receive flexible, trauma-informed, person-centred support when they need it, leading to increased periods of stability and more opportunities to make positive changes in their lives. Changing Futures is a research programme and therefore as well as delivering a frontline service with consent data is collected from the adults we work with to demonstrate how the approach works.Experience we are looking for:

  • Social Worker registered with Social Work England, please include your registration number on your application.
  • Completed your assessed & supported year in employment (ASYE).
  • Supervision skills.
  • Communicating with adults where there is complexity.
  • Managing workload independently.
  • Demonstrate effective judgement, identifying, assessing, and managing risk.
