Will relocateThe Kent, WA market is in need of a Home Health skilled RN-with Oasis experience we can do this several ways:JOB DESCRIPTIONREGISTERED NURSEREPORTS TO: Director of NursingJOB SUMMARY: Professional member of home health team who provides skilled nursing visits in patient’s home under the direction of plan of treatment established with physician and input from patient/care giver.DUTIES AND RESPONSIBILITIES: * Provides initial and on-going assessment of client needs using the OASIS data elements incorporated into the comprehensive assessment
- With input from patient/care giver and in conjunction with physician, formulates and implements plan of care
- Evaluate effectiveness of care plan and make necessary adjustments
- Provides for the emotional and physical comfort and safety of client taking into consideration their rights and cultural background
- Receives and transcribes physician orders
- Notifies physician and agency supervisor of unusual reactions and/or changes in client’s condition
- Documents all appropriate observations and treatments in keeping with agency policies and procedures
- Participates in case conferences, team meetings, staff meetings, and performance improvement activities as assigned
- Provide any skilled nursing service for which appropriately trained which is prescribed under plan of care. Prepares clinical progress notes as per agency policies and procedures
- Provide 60 day summary of skilled services and client outcomes to physician and agency supervisor in keeping with agency policies and procedures.
- Adheres to all agency policies and procedures including but not limited to the HIPAA privacy rule.
- Maintains strict confidentiality of all patient, employee and agency information
- Other duties as assigned by supervisor and for which he/she is qualified
- Conducts self in professional manner at all times and in all situations
- Participates in all agency in-service programs QUALIFICATIONS:
- Must be a graduate of an approved school of nursing
- Currently licensed in the state of the HHA
Contract for 13 weeks or Direct Hire
sign on of 10,000 or relocation fees and sign on of $5000(FT includes phone and internet allowance of $50 each per month, tablet for documentation, mileage of .50/mile)Start date asap
