The H.O.M.E Program provides a multi-disciplinary, multi-sectoral, mobile response to improve the health outcomes and health equity of highly marginalized individuals in London, Ontario who face barriers to accessing traditional models of care. Through an integrated mobile team, H.O.M.E aims to bring service to clients where they are at, including encampments, emergency shelters, housing, and other community settings. The AMHW strives to re-connect with clients, build relationships with those who may not be accessing services, and ensure the delivery and continuity of comprehensive primary care (via the mobile vehicle) and wraparound supports are available to those most in need. The AMHW also provides outreach harm reduction education and supplies, addiction services, basic needs support, and connections to other resources.Successful candidates must have a Police Vulnerable Sector Check completed in the last six months.   • What you’ll be making – Starting rate for this role is $26.74 per hour (Level 1 – Direct Service), with a wage differential, based on regulation ($29.57 per hour for Level 1 – Direct Service, Regulated):
  • Successful candidates who are registered with one of the designated professional colleges (e.g. OCSWSSW, CNO, COTO, CRPO, CPSO, or CPO) will receive the wage differential for regulation
  • Successful external applicants will start at Level 1, with grid progression based on seniority up to Level 8 ($33.90 per hour for non-regulated, and $36.71 per hour for regulated)
  • For internal applicants, grid level placement will occur as per the Collective Agreement
  • When you’ll work – 70 hours per bi-weekly period, Monday-Friday 8:30am-4:30pm
  • What your benefits will be – After a 90-day waiting period, will be enrolled in extended healthcare benefits
  • Your pension plan – Immediately enrolled in the Healthcare of Ontario Pension Plan (HOOPP)

HOW YOU WILL BE MAKING A DIFFERENCE:Assessment–In collaboration with the client:

  • Assess clients’ skills and abilities to meet basic needs
  • Assess social and environmental factors impacting the client
  • Assess the adequacy of resources/supports currently available to the client
  • Determine the impact of addictions and/or psychiatric symptoms on functioning
  • Identify risk factors related to client and staff safety as well as relapse
  • Assess barriers to successful community living (e.g. housing, treatment options, transportation, finances, etc.)
  • Obtain information, as needed, from other relevant sources
  • Ensure the assessment incorporates cultural and ethnic factors
  • Identify areas requiring additional assessment

Individualized Goal Setting and Rehabilitation Planning — Utilizing the information obtained through the assessment process and in collaboration with the client:

  • Assist the client to identify his/her personal goals and preferences and utilize standardized tool (GAIN, VI SPDAT, etc.)
  • Design rehab plans based on client needs, goals and preferences
  • Assist the client to navigate systems and connect to appropriate resources
  • Assist the client to explore options and consequences of different choices
  • Build on the client’s personal strengths, abilities and resources
  • Help clients work towards their vision of a positive personal future
  • Help clients meet basic needs
  • Work in partnership with the client in all aspects of planning
  • Identify intervention strategies to meet goals
  • Obtain necessary consents
  • Document and communicate the plan to all relevant stakeholders

Implementation of Rehabilitation Plan — Based on identified goals and intervention strategies:

  • Provide needed information and education about mental illness, coping skills, services, resources, and supports available
  • Utilize basic communication and intervention skills, supported by empirical validity, practice guidelines and relevant standards of care for different interventions
  • Encourage and facilitate personal growth and development towards rehabilitation and recovery
  • Motivate the client to learn new skills and behaviours
  • Build on client’s personal strengths and adjust interventions based on client response
  • Support the client to access needed resources
  • Assist the client to identify and connect with personal/natural supports
  • Meet with the client regularly and support the implementation of goals to improve safety and well-being

Service Coordination and Advocacy:

  • Promote decisions/actions that are consistent with a culture that values client-centred care
  • Demonstrate ability to deal effectively with sensitive issues with clients using a harm-reduction and trauma-informed approach
  • Support inclusion and equity by respecting others, embracing diversity and addressing oppression
  • Demonstrate an understanding of best practices related to supporting this demographic
  • Establish a caring and therapeutic relationship with clients; demonstrating trust, respect, and honesty
  • Have knowledge about community resources relevant to clients, including other mental health services, entitlement and benefit programs, (e.g. ODSP, Ontario Works, Coordinated Access, etc.)
  • Identify, develop and maintain good links with a wide range of community resources
  • Assist the client to obtain needed resources by educating and supporting the client to access resources, linking the client to appropriate services, advocating with or for the client when needed
  • Demonstrate knowledge of community resources and its place within the mental health care system
  • Assist in building positive working relationships across the service system

Crisis Management:

  • Have knowledge of range of crisis prevention and intervention approaches including: crisis prevention strategies, crisis planning, crisis intervention, risk assessment, relapse prevention strategies, crisis services and hospital services

WHAT YOU’LL NEED TO APPLY:Police Vulnerable Sector Check RequirementsSuccessful candidates must have a Police Vulnerable Sector Check completed in the last six months.   • Diploma or degree in a health or social service related discipline (primary proof of credentials will be required during the hiring process i.e. original degree/diploma or official transcript)
  • First-Aid & CPR
  • Valid drivers licence and access to a vehicle with adequate insurance to carry out job duties
  • Ability to attend work regularly

We prefer the following qualifications:

  • Experience working with clients with a serious mental illness and/or a substance use disorder
  • Knowledge of serious mental illness and its impact on clients
  • Knowledge of addictions and a trauma and violence informed approach
  • Excellent knowledge of community resources in London-Middlesex
