Job DescriptionVox Consultants are hiring for a Case Manager to join a London Borough on a Locum basis.You will be:
Joining the Transition and LD Adult Team as a Case Manager; your role will be to:

• Undertake back logs of reviews to ensure cases are properly reviewed.
• Carry out a range of Social Care tasks including screening and triage, initial assessment, support planning and case coordination.
• Working as part of a multi – disciplinary team of Social Workers/ Occupational therapists, case managers and support workers/advisers.

Essentials:

• Right to work in the UK.
• Qualification credit Framework level 3 (QCF) in health and social care.
• Experience of working with adults in Social Care/Health/Voluntary Community Sector setting.
