Detailed job description and main responsibilitiesThe ADAPT Pathway provides focused, therapeutic interventions to adults residing within the Borough who require care and treatment for anxiety, depression, affective disorders (without psychosis), personality disorders and trauma.The service aims to be flexible and accessible. The pathways of care aim to offer individuals, choice and involvement regarding their treatment, actively encouraging their participation in the planning and implementation of their care.A personalised care plan will be devised in consultation with the service user and their Carer/family where appropriate.Whilst Pathways of Care utilises a team approach to the delivery of care, every individual receiving care will be allocated either a health care professional (HCP) or care coordinator. This worker will be responsible for ensuring that care is delivered in accordance with the care/treatment plan, and that the needs of the individual are regularly reviewed, risks and care plans modified accordingly.Person specificationGMC Registration applicationEssential criteria
