About the Rifle VCLC

The mission of the Colorado Veterans Community Living Center at Rifle Colorado is to provide superior care to residents and to deliver exemplary service to those who reside in, visit, or work at the facility, in a manner that will enhance the quality of life, preserve dignity, and respect the rights of all. Professionally manage all aspects of the health information operations, quality of medical record documentation within professional practice standards set forth by the American Health Information Management Association, Colorado State Health Department Regulations, Federal and Veterans Administration Regulations and HIPAA Regulations

Description of Job

About the Position:

The Health Information Manager position exists to assume total and independent responsibilities for the organization and administration of the health information program for the entire facility. This position is involved in quality assurance of the medical records department; coordinates the Utilization Review Committee and maintains the minutes, serves as a member to several required committees as assigned; including the management team. Maintains the security, confidentiality, and accuracy of medical legally sound health records on each resident admitted to the facility as well as serves and a resource on medical-legal issues, policies and procedures, confidentiality, and HIPAA regulations. Conducts ongoing staff education in areas of documentation, HIPAA regulations and confidentiality. Job duties in this position include:

Health Information Management:

? Facilitates organization and administration of the Health Information program for the facility.

? In accordance with all regulations (State, Veterans Administration, Federal and HIPAA), initiates, maintains, safeguards, and promotes quality health records for each resident.

? Performs and/or delegates/oversees all health information functions utilizing a total working knowledge of manual and electronic health records, policies and procedures, medical terminology, coding and indexing procedures. Maintains design and revision of health record forms as necessary to comply with changes in State, Federal, Veterans Administration, and HIPAA regulations.

? Acts as resource authority for medical-legal issues, and documentation for any other departments within the facility to include the contract physicians.

? Reviews and analyzes health records for quality and quantity of health care professionals’ documentation in accordance with acceptable professional practice standards.

? Attend monthly triple check meetings to ensure all medical record documentation is correct, appropriate, and complete for billing purposes.

? Enters Medical Doctor’s diagnoses in accordance with International Classification of Diseases, Tenth Revision, Clinical Modification (ICD-10-CM) and procedures to ensure the quality of health records and health care data by verifying their completeness, accuracy, conformance to legal requirements.

? Calculates numbers for reports and determines the validity of authorizations for release of health information.

? Compiles new data to update resident demographics and medical diagnoses.

Medical Appointment Scheduling:

? Schedules Medical, Dental, Audiology, Vision and Mental Health appointments for residents within the facility.

? Determines what information needs to accompany resident to appointment and assembles consult packets for this purpose.

? Maintains 5 appointment calendars throughout the facility and documents appointments in Treatment record as well as in a tracking log.

? Notifies resident Medical responsible party of upcoming appointments.

? Follows up with physician offices to obtain reports from appointments for records.

? Ensures transportation to/from appointments is available

Physician Services:

? Coordinates the facility contract physician services including new resident assignments, admission history and physical appointments, monthly visits, completion of health records and associated forms.

? Acts as the liaison between physicians and the facility.

? Monitors physician services and documentation to ensure the effective delivery of patient care.

? Maintains credentialing and privileging of contract physicians; maintains copies of all licenses of physicians and staff, ensuring timely license renewal.

Management and Supervision

? Responsible for work lead of 1.0 FTE, including planning, organizing, managing and controlling all aspects of the medical records department.

? Develops annual department budget and follows established procurement rules.

? Manages performance planning and evaluations of the subordinate staff.

? Creates staff schedules to ensure coverage and good customer service.

? Develops plans of correction for any areas of non-compliance related to medical records management.

? Serves as a member of the Quality Assurance and Performance Improvement Committee reporting documentation concerns and possible corrections, identification of other facility, resident, staff, or visitor concerns.

? Serves as a member of the facility Leadership Team, participates in various committees and supports the Mission of our home and CDHS

HIPAA Security and Compliance Officer

? Manages requests for and the release of resident health record information to residents and/or families, other agencies, physicians, and insurance companies.

? Respond to subpoenas & make appropriate notifications to Nursing Home Administrator and VCLC Division Leadership.

? Establishes and maintains health record destruction schedule.

? Coordinates on-site pick-up monthly of information to be shredded by contracted company. Conducts annual facility HIPAA Risk Assessment to be reviewed by State HIPAA Privacy and Security Office.

? Maintains current knowledge of HIPAA regulations and changes. Conducts training once per year and during each New Employee Orientation to assure that staff are knowledgeable in the areas of HIPAA and confidentiality.

? Continually observes for potential problem areas of HIPAA compliance.

? Participate on the statewide HIPAA Steering Committee

Minimum Qualifications, Substitutions, Conditions of Employment & Appeal Rights

Minimum Qualifications:

Two years (2) of experience in an occupational field related to the work assigned to the position.

Substitutions:

A combination of related education, certification, and/or relevant experience in an

occupation related to the work assigned equal to two (2) years

HIMSS or equivalent certification will substitute for the required experience

Special Qualification:

Certification by the American Health Information Management Association

as a Registered Health Information Technician (RHIT), Or Registered health information administrator (RHIA.) If not currently certified, will obtain HIPAA certification within 9 months of hire

Preferred Qualifications:

  • experience in medical appointment scheduling

  • experience in records management

  • leadership experience

  • Certification as a Certified HIPAA Professional (CHP) is desirable, however, can be obtained after employment.

Conditions of Employment:

  • Background Check- ALL CDHS employees (non-direct contact):

CBI name check, ICON Colorado court database, Medicare fraud database, Reference checks, Professional License verification (licensure requirements), and drug screen (commercial Driver’s license)

  • Former State employees who were disciplinarily terminated or resigned in lieu of termination must disclose the information on the application and provide an explanation why the prior termination or resignation should not disqualify the applicant from the current position. Absent extraordinary circumstances, prior disciplinary termination or resignation in lieu of termination will disqualify the applicant from future State employment with CDHS.
