If you’re a motivated individual with a positive attitude, we want to hear from you!📝 Job Description:As an Occupational Therapist, you’ll plan and administer therapy treatment programs to restore function, relieve pain, and prevent disability following disease, injury, or loss of body parts. This role operates on a 9:00 AM – 5:00 PM schedule.🔍 Key Responsibilities:

• Evaluate participants referred by the provider and administer treatment programs in collaboration with the provider.
• Complete assessments, progress notes, treatment plans, discharge summaries, and interdisciplinary notes.
• Perform participant evaluations and treatments at the PACE site and in the field (e.g., participant’s home, nursing homes).
• Maintain a clean and safe environment, following safety protocols.
• Participate in developing participant care plans with other interdisciplinary team members.
• Document all pertinent participant information, including evaluations, progress notes, MDS, and care plans.
• Consult with physiatrists and other professionals when appropriate.
• Maintain high standards of health care and clinical practice through continuing education and professional development.
• Participate in the clinical education of student affiliates and provide clinical instruction to COTAs.
• Recommend equipment for discharges as appropriate.
• Engage in Performance Improvement Monitoring.
• Act within the scope of individual authority to practice.
• Meet standardized competencies established by the PACE organization before working independently.

💼 Essential Skills:

• Successful completion of an Occupational Therapy program from an accredited university.
• Bachelor’s Degree in Occupational Therapy.
• New York State licensure as a Registered Occupational Therapist.
• One (1) year of experience as an Occupational Therapist and/or experience with the Geriatric population preferred.
• Minimum of one (1) year of experience working with a frail or elderly population, or willingness to receive appropriate training from the PACE organization if less experienced.
• Ability to establish rapport with patients and staff.
• Proficiency in using all OT modalities and equipment.
• Ability to travel to participants’ homes, nursing homes, and various sites/clinics.

🌟 Additional Skills & Qualifications:

• Knowledge of PACE regulations.
• Familiarity with Care Compass and other EMRs preferred.

🏢 Work Environment:

• The work environment includes both the PACE site and field locations such as participants’ homes and nursing homes.
• The position requires traveling to various locations for participant evaluations and treatments.
• The Occupational Therapist will work in a collaborative and interdisciplinary team setting, adhering to safety protocols and maintaining a clean and safe environment.
• Professional attire is expected while on duty.
