The role will support the monitoring and sustained delivery of PSFU by measuring and evaluating the benefits.Facilitate cancer services by coordinating and supporting the governance and roll out of the remote surveillance by working across professional, departmental and Trust boundaries as required to develop and support patient-centred seamless care, with particular focus on the delivery of Personalised Care, patient self-management and wellbeing services.This role will allow for flexible working with allowance for some home working.The candidate should have experience of cancer nursing, transformation programmes and the implementation of digital platforms in cancer services.
