  • Conduct Comprehensive Assessments: Begin the therapeutic journey by performing psychosocial assessments for new patients, creating and updating detailed plans based on your findings.
  • Support and Educate: Maintain vital connections with clients’ families and significant others, providing essential education, support, and advocacy to enhance their understanding and involvement.
  • Coordinate with Community Resources: Act as a pivotal liaison with community-based social and mental health agencies, ensuring comprehensive support and seamless care transitions.
  • Discharge Planning: Take the lead in discharge planning and perform diligent post-discharge follow-ups. Your efforts are crucial in linking clients with community resources, significantly reducing the risk of re-hospitalization.
  • Facilitate Individual Therapy and Groups:
  • Maintain a caseload for weekly individual therapy and bi-weekly family therapy.
  • Lead therapeutic groups, providing a structured yet dynamic environment for client interaction and support.
  • Ensure Continuous Care: Our therapist’s role extends beyond discharge, as you will ensure clients continue to receive the support they need within the community.

Unrestricted licensure in social work required. LMSW or LPC (preferred).
