Join our team, and help recognize a healthy future for your career and for all Canadians.Why this role is important?Nurse Case ManagerNature and ScopeThe Nurse Case Manager will: provide virtual health coaching, Virtual health system navigation, disease and drug specific coaching, respond to patient inquiries, maintain a patient caseload, utilize effective communication skills, and effective health coaching and problem-solving abilities, both in a team environment as well as independently. Building strong working relationships both internally and externally are key responsibilities to meet specific team and business objectives. In addition, you will attend pre-booked training sessions, complete self- learning assignments and independent studies while adhering to SDM Specialty Health Network Standard Operating Procedures, Policies, and the College of Nurses Standards of Practice, and adhere to Shoppers Health Solutions guiding documents.What you will do:Responsible for assigned caseloadAct as the primary resource & contact for patients, within the designated programCreates effective client assessments, action plans and follows up for completion of coaching journey as requiredCoordinates & facilitates the client progress from enrollment to drug administration and throughout therapySupports Virtual health system navigation, client coaching for disease state queries, medication effectiveness questions,Facilitate self-care through the tenets of advocacy, shared decision-making, and educationPractice cultural competence, with awareness and respect for diversityEnsure thorough documentation in patient file to ensure continuity of care is maintained and that any changes in client care are clearly identified and transitioned accordinglyProvide status updates related to reimbursement; dispense and health coachingPromotes communication and coordination between the departments within SDM Specialty Health Network and Shoppers Health SolutionsRespect confidentiality, advocate for client to facilitate positive outcomes, and ensure that ethical issues are identified and resolved according to the Canadian NursesParticipate in internal and external committees as requiredPlan and implement in-services and continuing education programs for both internal and external stakeholders as requested by management.Demonstrate commitment to Continuous Quality ImprovementMaintain high standards of professional practice by adhering to the ethics of the profession and keeping abreast of the latest professional advances and Association Code of Ethics are maintainedParticipate in ongoing reflective practice activitiesAssume responsibility for personal and professional developmentMay be required to participate in on-call coverageOther responsibilities as required by SDM Specialty Health NetworkWhat you will needFive or more years direct clinical experiencePrevious experience working in a Virtual health care environment an assetCurrent professional nursing license in the province of work and willingness to secure additional licensure as requiredFluently bilingual in both written and spoken languages (French & English) is an assetBScN preferred, however, a combination of experience and education will be consideredAbility to provide an organized, efficient approach to case managementPossession of excellent skills in assessment, communication, critical thinking, and decision makingAbility to demonstrate understanding of SDM Specialty Health Network policies, legal considerations and professional ethicsProficiency of computer skills including Microsoft applications (Outlook, Word, Excel and Internet Explorer); experience with web-based applications (Salesforce)Ability to present to fellow colleaguesAbility to remain objective and be a team playerAbility to adapt to a changing environmentCommitment to the principle of quality client-centered service deliveryKnowledge of theory and implementation of Continuous Quality Improvement processesKnowledge of case management processesWhy work at Shoppers Drug Mart?Acting as consultants, supporting internal customers and the Associate-Owners, you will get experience unique to the retail industry while working for an award-winning national brand.
