– Application Deadline: October 31, 2024 –Reporting directly to the Manager, Addiction Medicine, the Nurse Practitioner (NP) works directly with the Youth Hub at the YOU (Youth Opportunities Unlimited) location. The NP is responsible to provide a wide variety of comprehensive care using advanced assessment skills and a sound knowledge of clinical therapeutics. Interventions encompass primary health care, health promotion and education, addiction support and treatment, advocacy, program development and evaluation. The NP provides support in collaboration with various inter-professional teams across the organization and community partners in order to support youth to improve overall health and well-being.Consider revitalizing your career and making a difference in your community by joining our team!WHAT WE’RE OFFERING:

  • What you’ll be making – Starting rate for this role is $64.72 per hour (Level 1 of the Nurse Practitioner Grid);
  • Successful external applicants will start at Level 1, with grid progression based on seniority up to Level 8, $69.82 per hour
  • Internal applicants will be placed on the grid based on their seniority
  • When you’ll work – 48 hours over two-week period, Hours will be between Monday-Friday 8:30am-4:30pm; There is some flexibility/ability to negotiate days of work for the week
  • Your benefits will be – After a 90 day waiting period, will be enrolled in extended healthcare benefits
  • Your pension plan – Immediately eligible for optional enrollment in the Healthcare of Ontario Pension Plan (HOOPP)

HOW YOU WILL BE MAKING A DIFFERENCE:

  • Establish a caring and therapeutic relationship with clients; demonstrating trust, respect, and honesty

  • Demonstrate ability to deal effectively with sensitive issues with clients and families using a harm-reduction and trauma-informed approach
  • Support inclusion and equity by respecting others, embracing diversity and addressing oppression
  • Demonstrate an understanding of best practices related to supporting a youth demographic
  • Collaborate with members of the interprofessional team, regarding care planning and follow-up for individuals with high risk or complex health concerns
  • Perform comprehensive care using advanced health assessment, physical examination, therapeutic management, consultation, collaboration and referrals in accordance to the Practice Standard: Nurse Practitioner CNO
  • Retain clinical competence through relevant evidence-based research and legislation
  • Demonstrate ability, commitment to and knowledge of community addictions and mental health care
  • Provide non-urgent, health care services within the NP’s full scope of practice to clients who encounter barriers to accessing care
  • Participate in the development of and work under current medical directives for the program
  • Exhibit knowledge of evidence-informed guidelines, legislation, regulation and organizational policies when prescribing controlled drugs/substances (CDS)
  • Commit to continuous learning through professional development, giving and receiving constructive feedback and applying new skills
  • Support the dissemination of research, knowledge and innovative practices
  • Demonstrate excellent communication, decision-making, problem-solving, conflict management and time management skills
  • Proficient in computer skills and experience with electronic medical record systems (i.e. PS Suites)

ADDICTION SPECIFIC COMPETENCIES:

  • Demonstrates an understanding of the neurobiology of addiction
  • Advanced practice assessments and interventions for a diverse range of clients with addiction problems
  • Assesses and manages patients with addictions who have co-existing health conditions including but not limited to mental health conditions, blood borne diseases and infections
  • Advanced health assessment to identify clients’ emotional, cognitive and behavioural states, as well as level of anxiety, crisis states, indices of aggression, self-harm, suicide, risk to others, competency to care for self, and signs of substance abuse, addiction, and withdrawal
  • Utilizes evidence‐based interventions, including withdrawal and other medication management, to meet the needs of a diverse range of clients with addiction problems
  • Provides care to persons experiencing an addiction and / or mental health condition that is recovery oriented, trauma-informed, uses principles of harm reduction, and addresses social determinants of health.
  • Engages clients in strengths-based care that promotes resilience
  • Provides education to individuals and families about addiction and its management
  • Facilitates and engages in collaborative, inter- and intra-professional, and intersectoral practice when providing care for persons with an addiction(s)
  • Understands stigma as a barrier and assists and supports patients and families
  • Demonstrates knowledge related to the process of voluntary and involuntary care
  • Acts as a resource providing addiction‐related clinical advice for health and other professionals, community groups and clients and their families
  • Advocates for persons experiencing an addiction

WHAT YOU’LL NEED TO APPLY:Police Vulnerable Sector Check RequirementsSuccessful candidates must have a Police Vulnerable Sector Check completed in the last six months. Interested applicants who reside in the City of London and are in need of a Police Vulnerable Sector Check (PVSC) are encouraged to apply for one as soon as possible as PVSCs in the City of London are currently taking approximately 2 weeks to be processed and offers of employment cannot be made without receipt of a PVSC.We require the following qualifications:

  • Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class (primary proof of credentials will be required during the hiring process i.e. original degree/diploma or official transcript)
  • Current certificate in cardiopulmonary resuscitation (CPR)
  • Current vulnerable sector police check
  • Demonstrated ability to attend work regularly

We prefer the following qualifications:

  • Master’s Degree (Preferred)
  • Minimum of two to three years of community health care and/or primary care experience
  • Experience in addiction medicine and harm reduction
  • Excellent clinical, interpersonal and organizational skills
  • Project planning and evaluation skills would be an asset
