Job Description

Assesses, plans, coordinates, implements, and evaluates education for Behavioral & Mental Health Case Management team which promotes clinical and professional development resulting in improved quality of care for patients and families. Performs duties associated with clinical practice, professional development, leadership and quality, and evidence-based practice. Develops and implements education plan for assigned area to proactively support delivery of safe patient care.

This role will serve as both clinical educator/case manager. Schedule is 4 10-hour weekdays. Will require travel to hospital facilities as needed.

Experience

  • 5 years of clinical experience in case management

Preferred Qualifications

  • Nursing Professional Development (NPD) certification
  • Previous adolescent/adult mental health experience helpful
  • Previous experience in clinical education, training, charging

Education

  • Bachelor of Science in Nursing (BSN)

Certification Summary

  • Current Georgia license as a Registered Nurse
  • Basic Life Support (BLS) from the American Heart Association within 30 days of employment
  • Nursing Professional Development (NPD), Certified Case Manager (CCM), Case Management Board Certification (CMGT-BC) or Psychiatric-Mental Health (PMH-BC) certification within one year of hire

Knowledge, Skills, and Abilities

  • Excellent communication, organizational, and interpersonal skills
  • Strong presentation skills to showcase credibility and effectiveness
  • Ability to organize workloads and resources
  • Basic knowledge of adult learning principles in the design of educational activities to provide quality training materials
  • Proficiency in using professional technology tools such as Microsoft Office Suite, Workday and Learning Management System (LMS)

Job Responsibilities

  • Facilitate onboarding of new employees which includes the process of orienting, socializing, and integrating an employee to the organization with a focus on retention and growth.
  • Provide ongoing education and refreshers related to case management, community resources, and care coordination.
  • Educate on department specific competencies, including but not limited to, utilizing technology platforms (i.e. CSP Board, Xferall, etc.), facility referral and consent process for acute and long-term placements, transportation initiation (ground & air), prior authorizations, securing durable medical equipment, completing letters of medical necessity, etc.
  • Educate on department specific policies and actively participate in review and revision of policies and procedures.
  • Develop, coordinate, manage, evaluate, and document expected outcomes and objectives to support the dynamic process of competency assessment, learning, and change.
  • Promote compliance with regulatory bodies standards, including the creation and maintenance of educational records.
  • Utilize novice to expert continuum with developing employees.
  • Act as clinical resource for patient population and maintains ability to function as BMH Nurse Case Manager.
  • Align educational programs with organizational and strategic plans to include quality and safety trends.
  • Conduct learning needs assessments and practices gap analyses through environmental scanning to achieve specific outcomes in identified deficits or opportunities for improvement in knowledge, skill, or attitude.
  • Identify desired outcomes and plan, design, and deliver learning activities using adult learning concepts and instructional design principles to achieve specific outcomes for identified target audiences, which may include license and non-licensed healthcare personnel.
  • Evaluate learning outcomes and revise educational activities and orientation programs based on data and feedback.
  • Monitor resource allocation and utilization by considering cost-effectiveness of educational activities.
  • Document provided educational activities using LEAN methodology and Children’s current Learning Management System (LMS). Demonstrates proficiency in the use of Children’s LMS.
  • Advance professional development by identifying and developing strategies to facilitate a continuous process of role maturation for self and others.
  • Foster a culture of lifelong learning and professional development through role-modeling, mentoring and guidance.
  • Promote growth and development of leadership skills, accountability, clinical decision making, and problem-solving within self and others.
  • Maintain clinical knowledge, skills and competence in the patient care setting as evidenced by the ability to deliver proficient age and developmentally appropriate safe patient care.
  • Engage in organizational committees (i.e. TEACH) and lead a shared governance council.
  • Introduce and support new ideas and initiatives, exhibit creativity and flexibility in times of change.
  • Partner with leadership team to provide performance feedback, input and coaching during orientation and beyond to guide and direct employee role maturation and career development and advancement.
  • Incorporate research, evidence-based practice (EBP), practice-based evidence and benchmarked best practices into educational planning and to guide practice decisions.
  • Participate in development and implementation of departmental process improvement plans and quality measurements.
  • Collaborate with partners and stakeholders to share expertise in planning and decision-making to achieve desired results.
  • Support academic and community partnerships.
