The Medicine for the Elderly clinical pharmacy team is pivotal in preventing hospital admissions and enhancing care throughout the patient pathway. Frail patients, often facing complex health issues and medication regimens, require specialized expertise in medication management. Clinical Pharmacy brings a unique skill set, ensuring that medications are not only optimised for efficacy but also tailored to the specific needs of frail individuals. By conducting comprehensive medication reviews, identifying potential adverse reactions, and collaborating closely with interdisciplinary teams, the clinical pharmacy team plays a crucial role in mitigating medication-related risks. This proactive approach not only prevents unnecessary hospital admissions, reducing the burden on healthcare resources, but also contributes to an overall improvement in the quality of care for frail patients.We are seeking an exceptional candidate to be a Specialist Clinical Pharmacist, working as part of the clinical pharmacy team to develop services across the patient pathway to ensure continuity of care, promote better health outcomes, and fostering a patient-centred approach that addresses the nuanced challenges faced by frail individuals in their healthcare journey. A key component of your role will be to carry out polypharmacy reviews in collaboration with medical colleagues effectively utilising your clinical knowledge & skills and prescribing qualification to positively contribute to patient care.You will work at a senior level within the service and play an influential role in the patient’s care pathway whilst building upon proven clinical skills. The combination of responsibilities will significantly facilitate your growth in both the clinical and leadership aspect of this role.You will have previous experience of working within a clinical environment and will have completed a post-graduate diploma in clinical pharmacy. Evidence of multi-disciplinary teamwork in Elderly Care and well-developed communication and leadership skills would be advantageous.Training and support will be provided. A commitment to continuing professional development is essential. This reduction will also be applied pro rata for part time staff. This advert and any subsequent offer/contract of employment therefore reflects the new working hours. However, as not all service areas will be able to adopt the 37 hour working week immediately from 1 April 2024, you may be required to work up to an additional 30 minutes per week for a temporary period for which you would be paid until the service you are working in changes rosters or working patterns to accommodate the new reduced working week.
