• Assist the patient with transfer, ambulating and prescribed exercise and instructed by the Home Care Nurse.
• Prepare modified diet as ordered by the Home Care.
• Observe and report specific vital signs and symptoms as instructed by Home Care.
• Remind the patient to take medications daily.
• Monitor skin changes daily, monitor changes and report to Agency RN.
• Perform incidental household functions necessary to maintain a safe E.g. comfort, cleanliness of the patient, and grocery shopping.
• Report changes in the patients’ physical status to the Home Care Nurse or Home Care.
• Submit annual mandatory compliance documents such as annual TB questionnaire.
• Completes 12 hours of mandatory annual compliance training.

QUALIFICATIONS * Must be a certified Home maker/Home Health Aide.

• Be a minimum of 18 years of age, able to read and write English.
• Bi-lingual is a plus.
• Have had a physical examination satisfying the New Jersey State Department of Consumers Affairs requirements for Home Health Care Workers.
