As a Team Leader you will support carers and seniors across our north and south teams, ensuring we provide the best service possible. You will also:Assess, plan, implement and evaluate care plans with clients and team membersDeal with new referrals into the serviceLiaise and work in partnership with the multidisciplinary teamPromoting personalisation in care and maximising independence for customersCarry out supervisions, appraisals, and mentoring to colleaguesProvide personal care to clients where neededBe required to be on-call on a roster basis – there is a financial incentive for this responsibilityWorking every other weekendAbout youAn experienced and confident leader, you will have a background in providing care. Ideally, you will have some experience of leading a team either as a senior or team leader but this isn’t essential.You’ll need to have a driving licence and access to a vehicle so you can travel to our clients. Alongside this you will be a clear and confident communicator whether that is with colleagues, clients or health professionals.About UsOur Somerset Community team covers the areas of Bridgwater, Highbridge, Burnham-On-Sea, Cheddar and surrounding areas with teams based in each of these areas. The team are passionate about supporting clients to remain as independent as possible within their own homes.The team is led by Mandy, Operations Manager, who is happy to talk through any of the roles available.
