The salary range for this position is CAD $49.02/Hr. – CAD $61.21/Hr.Job SummaryCome work as a Social Worker with Vancouver Coastal Health (VCH)!Vancouver Coastal Health is looking for a Social Worker to join the Home Health team at South Community Health Center in Vancouver, BC in a team leader capacity. Apply today to join our team!As a Social Worker with the Home Health Team you will:

  • Oversee and coordinate the delivery/daily activities of designated programs/areas, evaluate services to ensure program/service needs are met, and liaise with key partners on the achievement of program goals and objectives.
  • Promote interdisciplinary practice and fosters a safe environment for staff and clients.
  • The role provides leadership for the delivery of a service, working in partnership with the organizational resource areas, including Professional Practice, Clinical Education, Clinical Quality and Safety, and other organizational key partners, to ensure a high standard of care founded in best practice and standards of care.
  • Provide supervision, to at least one (1) other health science professional and a total staff of more than sixteen (16) FTE which may include case managers (Social Work, RN, LPN, & Rehab) and/or support staff such as Community Liaison Workers.
  • Provide continuous communication, conflict resolution and mediation, performance recognition, coaching, performance management, safe work environment, facilitates client transitions, and manages workload and staffing issues.
  • Support the Manager with programs related to attendance wellness promotion, respectful workplace, accommodation, recruitment, hiring, selecting, mentoring and retaining staff.
  • Participate with clients, family, community support and interdisciplinary team in case management meetings and maintaining related client records.
  • Provide advanced clinical practice support and guidance to the interdisciplinary team to address challenges and barriers related to complex psychosocial factors and social determinants of health.

ABOUT HOME HEALTH PROGRAMS:Home Health programs provide a range of health care and support services to people living in the community who have acute, chronic, palliative or rehabilitative health care needs.THE CLIENT POPULATION:Home Health clients typically have on-going health issues and/or complex personal and/or social issues, requiring care planning and coordination of appropriate resources. Clients include frail elderly, younger clients with disabilities, and adults with chronic illnesses requiring supports to maintain/promote independence. South serves the southern part of the city including Fraserview, Killarney, Oakridge-Marpole and Sunset neighbourhoods.QualificationsEducation & Experience

  • Current full registration with the British Columbia College of Social Workers.
  • Master of Social Work from an accredited School of Social Work. Five (5) years recent related clinical experience working with individuals with complex care needs, including two (2) years supervisory and/or leadership experience or an equivalent combination of education, training and experience.
  • Valid B.C. Driver’s license and local area travel requiring the use of a vehicle may be required.

  • Broad knowledge of systems theory and its clinical application in social work process with designated population(s).
  • Broad knowledge of other health disciplines and their role in healthcare.
  • Broad knowledge of case management and social work practice within a client and family-centred care model for clients with complex health care needs.
  • Knowledge of appropriate Acts (e.g. Adult Guardianship, Mental Health Act) and legislation as it relates to the rights and obligations of clients and staff.
  • Broad knowledge of counselling skills and clinical practice models.
  • Demonstrated ability to provide competent and culturally safe care in a variety of settings and with diverse populations
  • Comprehensive understanding of trauma-informed care.
  • Comprehensive knowledge of Social Determinants of Health and its implications to care provision.
  • Demonstrated knowledge of adult education principles and ability to effectively teach and lead group facilitation.
  • Knowledge of the mandates and policies of other government agencies including Home and Community Care, Ministry of Human Resources and Public Guardian and Trustee.
  • Knowledge of cultural diversity in relevant healthcare populations, and culturally safe ways of engaging.
  • Demonstrated ability to conduct social and psychosocial assessments.
  • Demonstrated ability to understand and influence group and inter-group processes, decision-making and problem solving.
  • Demonstrated ability to establish rapport with clients and families in order to explore issues that are beyond the initial presented concerns.
  • Demonstrated ability to adjust to new or unexpected events, problem solve, and deal effectively with and/or guide others in resolution of conflict issues.
  • Demonstrated ability to communicate effectively, both orally and in writing, with clients and their families, colleagues, physician, and other health care staff, both one-on-one and in groups.
  • Demonstrated listening and information seeking skills that promotes open and direct communication and leads to a cooperative approach to problem solving.
  • Demonstrated ability to work effectively with community partners and with individuals from diverse backgrounds.
  • Demonstrated ability to operate related equipment.
  • Physical ability to perform the duties of the position.
  • Basic computer literacy to operate a computerized client care information system and word processing, spreadsheet, Internet and e-mail software.
