This program operates out of the West Haldimand General Hospital.is a non-profit organization providing personal support services to adults with disabilities living within their own home or one of our independent living facilities. Our clients direct and customize the services they receive and we support them to maximize their independence and enhance their quality of life. Assists with the development and implementation of admission and discharge protocols and acts as the liaison with community partners, hospitals and next-level care providers to ensure seamless client-centred transitions into and out of service locations. Responsible for developing Service Agreements and Transitional Plans with clients, primary care givers and health care partners in accordance with regulations and best practice approaches in transitional care. The position facilitates the accurate sharing of client-specific information between health care providers and care teams to ensure continuity of care, client safety and well-being. The position assists with the review of nursing standards and protocols; providing in-service training and daily coaching to healthcare team to ensure application of same. During times of insufficient staffing may be required to provide direct nursing care.Employment GuideCompletion of a vocational, college or other approved program for licensed practical nurses and registration with a regulatory body required. Must have successfully completed the Canadian Practical Nurses Registration Examination and possess a current Registered Practical Nursing Certificate of Competence for the Province of Ontario. Prior experienced in geriatric, rehabilitation, or working with adults with disabilities an asset. Ability to work within legislation pertinent to the operations of a health care facility including but not limited to such acts as Public Health, Mental Health, Resident’s Protection and other federal, provincial and municipal statutes with operation of Nursing Services in community care. Must demonstrate friendly, professional and business-like demeanour and have a sound understanding and empathy for the needs of older adults and their primary care givers. Prior knowledge of using a variety of health care data management software desirable. Must be able to communicate effectively both verbally and in writing. Must demonstrate the ability to use tact and discretion and display professionalism at all times, demonstrate sound judgement and flexibility and be able to solve problems effectively. Must be a strong team player with the ability to coach and inspire others. Must be able to meet physical requirements of the job such as lifting and transferring of clients. A valid driver’s licence and reliable vehicle are requirements of this positon.
