Key Responsibilities:

• Administer medications & treatments as prescribed and track patient progress
• Monitor vital signs & provide personal care in line with Individual Service Plans (ISP)
• Coordinate medical care, including scheduling, referrals, and follow-ups
• Manage medical supplies & ensure proper inventory for patient needs
• Respond to emergencies with CPR, first aid, and other life-saving techniques
• Train & support direct care staff on medical procedures and equipment use

Requirements:

• Active LPN License in MA
• At least 3 months of direct care experience (healthcare, group home, or residential setting)
• Basic Life Support (BLS) Certification (or willingness to obtain)
